How long to continue PCT on a Testosterone cycle?

I am about to do my first cycle 300-350mg Test E for 12 weeks and I read alot online about what I need to do during the cycle as well as post cycle, however I cannot find how you know if your PCT should last 4 weeks or if you have to extend it to 6 or even possibly more. Also once I get blood work a few weeks after PCT is done and my levels aren't where they should be do i continue to take nolvadex and clomid?
